Volunteer Information
Most volunteer activity at Maxwell Elementary School is coordinated through the PTA. The PTA Volunteer Coordinator is responsible for maintaining a list of active volunteers, soliciting volunteers for different activities, and recording volunteer hours worked. A Volunteer Information Sheet is available upon request.
All questions, comments or suggestions about volunteering, or about this page of the website, should be directed to the Volunteer Coordinator. Written correspondence or forms may be delivered to the Volunteer Coordinator by placing them in the PTA mailbox. This is located in the mailroom in the front hallway, past the front office and right next to the water fountain. The PTA mailbox is in the upper right-hand corner as you face the wall of mailboxes.

How to Become a Volunteer
Becoming a volunteer at Maxwell involves two easy steps. First, you fill out a short form allowing the district to request a background check in Frankfort. This application is now available online! This check will determine if there is a criminal record that would prevent you from volunteering in the school system. You only need to fill out the form every five years and be sure to check all schools in which you would like to volunteer.
This form is sent out to each family at the beginning of every year, and additional copies are available in the front office. Or, click here to get a pdf version of the form. The completed form should be returned to the Principal, who will send it in for processing. Please note that each parent or family member wanting to volunteer needs to fill out his/her own form.
Second, you fill out a one-page form indicating your desire to volunteer,
your availability, the sorts of volunteer work you’re interested in, and your
contact information. Two copies of this form are sent out to each family at the
beginning of every year. Additional copies are available from the Volunteer
Coordinator, or at this link: Volunteer Form.
The completed form should be returned to the Volunteer Coordinator. Once your
completed form has been received and recorded, you will begin receiving notices
of volunteer opportunities.
Recording Volunteer Hours
Recording the volunteer hours worked at Maxwell Elementary School is an important step for several reasons. The school logs the hours to show parental involvement. It goes on the school report card, is reported to the district office, and hours logged can count as “matching funds” for grants. We are taking steps to make sure that none of the volunteers’ work goes unrecorded.
Many volunteer hours are counted through the Visitors' Book in the front hallway. When you come into the school, you are expected to sign in the Visitors' Book. The sign-in page provides spaces for you to indicate whether you are there as a visitor or as a volunteer. There is also a space for you to indicate the amount of time you worked as a volunteer. The Volunteer Coordinator periodically goes through the Visitors' Book and adds up all the recorded volunteer hours.
Log Forms
What about volunteer hours worked outside of the school? Many volunteers do work at home, or running errands for various projects. We have two different log forms for these situations, one for frequent volunteers and one for occasional volunteers. If possible, please return your completed log forms to the Volunteer Coordinator at the end of every month.
Hard copies of the log forms are available in the mailroom/volunteer workroom, in marked drawers on the right-hand side of the room. They are also available as pdf files. If you would like to print out a form, please click on the link in the list below. If you would prefer to receive a form as a Microsoft Excel file, please email the Volunteer Coordinator.
- Volunteer Hours - Log Sheet: This is for individuals who do a fair amount of volunteer work outside the school, and would like the convenience of recording several instances on a single sheet of paper.
- Short Form: This is for individuals who do occasional volunteer work outside
the school. It's printed on a partial piece of paper, and will take about 15
seconds to fill out.
